Transcript Requests

We are a state agency and the official custodian of records.  If you have any questions, please email DPOS@dhe.state.co.us

DPOS retrieves student transcripts when a Board-approved private occupational school ceases operations in Colorado. To request a transcript or an education verification, please complete and submit this Online Student Record Request form to DPOS. The DPOS transcript request or education verification fee is $20.00 (non-refundable) NOTE: if requesting more than one copy, an additional $5.00 fee per copy is required.

As of August 4, 2026, all transcripts for the following schools have been transferred to Verif-y. All transcript requests or education verifications must be submitted using this link: https://transcript.verif-y.com/

If you submitted a request to DPOS previously for any of these schools listed below and have not received a response from DPOS yet, please submit a new request to Verif-y for processing.
